What to do, in order

  1. Get the discharge plan in writing. Ask the case manager or social worker for it on paper.
  2. Confirm inpatient versus observation status. Ask directly, and ask in writing. Medicare's skilled nursing benefit requires a qualifying inpatient admission, and observation nights do not count no matter how many were spent in a hospital bed.
  3. Ask which facilities actually accepted the referral. Case managers send inquiries to many communities and hear back from few. The list of places that have said yes is the only list that matters today.
  4. Push on the discharge date. It is usually less fixed than it sounds, particularly when no safe plan exists yet.
  5. Verify the license before you sign. A few minutes on your state's licensed-facility search tool, even inside a 72-hour window.
